What Is An Annular Tear?

An Annular Tear is a split or disruption in the annulus fibrosus, the tough outer ring of a spinal disc. The injury may allow the disc’s inner material to press toward the outer edge without creating a full disc herniation. Annular tears may occur in the lumbar or cervical spine and can contribute to localized pain.

Key Takeaways About Annular Tear

  • The injury affects the disc’s outer ring.
  • A tear does not always mean the disc has herniated.
  • Disc bulge and annular tear describe different findings.

How Annular Tear Works

Annular Tear describes damage within the annulus fibrosus, which surrounds the softer center of a spinal disc. Repeated loading, sudden strain, or age-related changes may create a fissure in these fibers. The fissure can irritate nearby tissues, and disc material may move toward the damaged area.

Clinicians may identify an annular tear through a combination of symptoms, physical findings, and imaging reports. A disc bulge involves outward extension of disc tissue, while a tear describes damage to the disc’s outer ring. A disc herniation may occur when inner disc material passes through a damaged portion of the annulus, but the findings do not always appear together.

From the Arrowhead Clinic Chiropractor Atlanta team

When we review an annular tear, we consider the reported symptoms, movement patterns, examination findings, and available imaging together. We use the finding to understand the broader disc condition and to guide care within our Herniated Disc Treatment service. We also distinguish the tear from a bulge or herniation.

Annular Tear in Practice: A Real-World Example

A person with lower back pain may have an imaging report that identifies an annular tear near a lumbar disc. The finding helps organize the evaluation, especially when symptoms change with sitting, bending, or other spinal movements.
